  1. Visit the DMV website to download and print form HSMV 82101, Application for Duplicate or Lost in Transit/Reassignment for a Motor Vehicle, Mobile Home, or Vessel Title Certificate.
  2. Complete the forms. Make sure that you have answered every question, that there are no blank spaces, and that you have signed and dated the application in the appropriate places. All owners must sign the form.
  3. Submit the form to your local Tax Collector’s Office in person or by mail.
